Vast disconnect between senior management and front line employees Poor middle management, benefits are too good for middle managers so there is little incentive for them to challenge status quo or move on and create opportunities for others Attitudes towards flexible working and employee wellbeing best described as "Victorian" Priorities and targets often change week to week with little communication. HR will sell this in as being a "fast paced" environment but that's slang for "no clear communication of targets or priorities" Real evidence of blame culture across a lot of departments which comes from the top down
