Antiquated IT systems - not current with the industry standard. Very high management turnover in US and Germany creates frequent changes to processes, procedures. New managers initiate "new ideas" that frequently are the same ideas that former managers implemented with no success. Although management preaches accountability, it is only lip service as there are no consequences for poor performing employees. Instead their workload is passed to A performers resulting in watching the best people getting discouraged and leaving the company.
