Lack of executive management intervention to ensure cross functional collaboration. A certain amount of butt covering is expected at every company but when it results in losing revenue through fear of highlighting and addressing systematic problems then you've a serious problem on your hands...something that Apple will address immediately. There is no work, life balance and bullying is the management style of choice, huge staff turnover which means they have to hire overpriced consultants to do the work their employees got sick of doing ; staff member from LA sent to Dublin to set up international office had all 4 direct reports resign in less than 10 months.
