Submitted online application and got called for an interview. They still had me fill out a long application when I got there. I performed a 50-minute Swedish massage with some deep tissue work on the assistant manager. I then had an interview with her and the manager together. First they outlined the company and the store's policies, asked me some questions about myself, my education and background, what I wanted out of the job, etc.

she wanted to really "get to know me" so I'd be prepared to talk about yourself and your personal background for a few minutes
Was asked to do a 50 minute practical on the manager with a small sit-down interview after. The lead therapist showed up briefly but didn't say too much to me; just kind of listened in. The manager asked me personal questions about my education and homelife and was encouraging me to work full time though I was available for part time.

Received phone call with brief interview and scheduled time to come in for hands-on evaluation where was able to continue dialogue/tour location and perform demo.
Demo is set up to how you would massage there so a standard 1 hour session break up:
5 min intake + medical history
50 min hands on
5 min outtake
Demo went well and offered job on spot though I asked a few days to decide.

Interview
Perhaps my situation was unique. I simply walked in unannounced to check the place out, knowing I wanted to apply. The manager happened to be there and we talked for at least a half hour. I told her of my desire to work there, asked her a few questions, and told of my experience and background in massage. It was an enjoyable conversation and she helped provide me with some resources regarding transferring my license. We scheduled an appointment for an official interview and practical (massage), which was today. Having spoken at length before, there were just a few questions but mostly by me about how they do things.
