Shoddy maintenance, anti-pilot management/administration, long duty days with not always enough flight time (lots of waiting in the heat), pressure from management to get flights done in bad conditions (bad weather, pilot fatigue, aircraft lacking maintenance constantly breaking and being pressured to go anyways).
