I pointed out that this does not happen if you follow the design process correctly. By getting a good brief, discussing and developing the concept, and then visualizing for approval... BEFORE undertaking lots of hours of artwork. It was like the interviewer had no idea how the design process actually worked.
Communication, first and foremost. If it doesn't get the message across it fails no matter how pretty it is. Secondly, reaching the right audience. Third, positioning the business/company effectively at the desired level.
