Abrupt changes, policies implemented without giving staff much notice. General hierarchical issues, right hand doesn’t know what the left hand is doing. Company culture lacks warmth and friendliness.

Focus on changing systems to better facilitate the detection and elimination of errors, inefficiencies, pain points, etc. Foster collaborative work environment by promoting intrinsic motivation instead of stick/carrot incentive structures.
